Community Resource Coordinator Opening, Albuquerque, NM

The Community Supports Coordinator is a full-time role with a standard schedule of 8:00 a.m. to 4:00 p.m., including a one-hour lunch break. The primary responsibility of this position is to support Plan Administrators in maintaining member compliance related to level of care, authorizations, and eligibility. This role is integral in the development and maintenance of Individual Plans of Care (IPoCs) for all members. Additional responsibilities include record keeping, compliance monitoring, and communication management with Managed Care Organizations (MCOs) and other stakeholders.
Key Responsibilities:
  • Assist Plan Administrators in ensuring member compliance with regulatory requirements.
  • Develop, update, and maintain Individual Plans of Care (IPoCs) for members.
  • Facilitate communication between members, MCOs, and healthcare providers (physicians, nurses, case managers, etc.).
  • Coordinate new member onboarding and ensure service availability.
  • Prepare and submit monthly reports, track compliance metrics, and file incident reports as necessary.
  • Support member care coordinators with supervisory visits and appeals processes.
  • Manage all correspondence with internal and external stakeholders, ensuring timely and accurate communication.
Administrative Duties:
  • Provide administrative support to the Director of Community Supports.
  • Organize meetings, manage calendars, and coordinate schedules.
  • Maintain accurate records of plan changes, updates, and any revisions.
  • Address inquiries and resolve issues from members, providers, and other stakeholders.
  • Collaborate with internal departments to ensure seamless plan administration and delivery of services.
Qualifications:
  • Proven experience in an administrative role, preferably within a healthcare setting.
  • Strong organizational and time management skills with the ability to multitask and prioritize effectively.
  • High attention to detail and accuracy in data entry and record-keeping.
  • Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook).
  • Excellent written and verbal communication skills.
  • Ability to work independently and as part of a team.
  • Familiarity with healthcare regulations and compliance standards is preferred.
Physical Requirements:
  • Ability to stand, walk, stoop, bend, and sit for extended periods of time.
  • Must be able to read, write, and communicate effectively.
  • Manual dexterity required for utilizing keyboards and office equipment.
  • Occasionally may need to lift up to 20 lbs. for filing and storing office materials.
